Zusammenfassung.
Ziel der Arbeit: Vergleich der Wirksamkeit einer interstitiellen Lasertherapie und einer Gleichstromtherapie
zur Behandlung von Lebermetastasen kolorektaler Karzinome im Rattenmodell. Methode: Bei 49 Ratten wurden Lebermetastasen durch subkapsuläre Injektion von Zellen einer
kolorektalen Tumorzelllinie implantiert. Nach 3 Wochen wurden 41 Tumoren mittels Lasertherapie
(2W) und 56 Tumoren mittels Gleichstromtherapie (80 C/cm3 ) behandelt. 17 Tumoren dienten der Gleichstromgruppe, 12 Tumoren der Lasertherapiegruppe
als Kontrolle. Die Angehrate sowie der Größenverlauf der Tumoren 3 und 5 Wochen nach
Therapie wurden mit einem 1,5-T-Ganzkörper-MR-System kontrolliert. Ergebnisse: MR-tomographisch ließ sich zeigen, dass aus 151 Tumorzellinjektionen nach 3 Wochen
133 Tumoren gewachsen waren (Angehrate 88 %). Nach der Lasertherapie betrug die mittlere
Größenzunahme des maximalen Tumordurchmessers 15 % nach 3 Wochen und 18 % nach 5 Wochen.
Verglichen mit der Kontrollgruppe (230 % und 280 %) war das Größenwachstum signifikant
geringer (p≤ 0,01 und p≤ 0,001). Nach Behandlung mit Gleichstrom betrug das Größenwachstum
40 % bzw. 60 % und war damit auch hier signifikant geringer als in der Kontrollgruppe
(210 % bzw. 290 %; p< 0,001). Schlussfolgerungen: In dieser tierexperimentellen Studie konnte erstmals eine Verringerung des Wachstums
von Lebermetastasen kolorektaler Karzinome durch die Applikation von Gleichstrom nachgewiesen
werden. Im Vergleich hierzu war der tumorinhibierende Effekt der Laserapplikation
jedoch ausgeprägter.
Local Therapy of Colorectal Liver Metastases: Laser vs. Direct Current Therapy in
a Rat Model.
Purpose: Evaluation of the efficacy of direct current and laser therapy in the treatment of
colorectal liver metastases in a rat model. Materials and Methods: Liver metastases in 49 rats were induced by subcapsular injection of cells of a colorectal
tumor cell line. 3 weeks after inoculation 41 tumors were treated by laser and 56
tumors by direct current therapy. 17 tumors served as a control in the direct current
group, 12 in the laser therapy group. The inoculation rate and the tumor size 3 and
5 weeks after therapy were determined by MRI. Results: The MR-tomographically determined inoculation rate was 133 tumors after 151 tumor
cell injections (88 %). Three and five weeks after laser therapy, tumor growth rates
of 15 % and 18 % were documented (maximum tumor diameter). This represented a significant
reduction in growth (p ≤ 0.01 and p ≤ 0.001) compared to the control group (growth
rate 230 % and 280 %). In the direct current therapy group, growth rates were 40 %
after 3 and 60 % after 5 weeks. Compared to the control group (210 % and 290 %), this
also represented a significant reduction (p ≤ 0.001). Conclusions: This animal study is the first to demonstrate an antitumor effect of direct-current
therapy on liver metastases of colorectal cancer in rats. Nevertheless, the laser
therapy proved to be more effective.
